Windows to Health is a survey of Baltimore City residents being conducted by the Johns Hopkins University. Yours and about 1000 other households in Baltimore City were chosen at random for this survey. We are inviting you to participate in this research because we would like to include your answers in the information we gather about health experiences in Baltimore City. We would also like to contact you again in the future for a more in-depth survey about health experiences in Baltimore City.
We are doing this survey as a first step to better understand what Baltimore City residents what need to be as healthy as possible. Your answers and the answers of the other participants will be combined so that we can understand more about health and the health experiences of people living in households across Baltimore City.
Households were randomly selected to participate in the survey. Only the households we choose are eligible to participate. People from other households in your neighborhood cannot participate unless they have been selected by us to participate. If your household was selected, for now we will only interview the head-of-household. The head of household is an adult who is responsible for the financial, physical, or day-to-day operations of the household. If he/she is unavailable, or too busy to complete the survey, the head-of-household may decide to have another adult living in the household do the survey instead.
Each person/HOH that completes the survey will be paid $10.
You may have received a card like this in the mail. This means your household has been selected to participate in the survey. If you would like to find out more about the survey and see if you are eligible to participate, you can:
1. Call 410-347-3210 and schedule a time for an interviewer to come to your home to interview you.

2. Call 410-347-3210 and schedule a time to do your interview by phone.
If you join this study, we will ask you to do the following:
1. Participate in a short (15 minute) survey about health experiences, called the Windows to Health Survey.
2. Provide us with basic information about you and the other people living in your household, using our Household Census Form. This should take 5 minutes or less.
3. Provide us with contact information that we can use to get in touch with you in the future using our Follow Up Information Form. This should also take 5 minutes or less.
We estimate the total time needed for your participation is about 25 minutes.
Every part of this survey is voluntary, and at any time, you may refuse to answer any question that you do not wish to answer.
